Zustand: Sehr gut | Sprache: Französisch | Produktart: Bücher | Depuis 2013, un traitement court de 9 mois de la tuberculose multi-résistante (TB-MR) a été introduit dans le système sanitaire ivoirien. Du 13 novembre 2013 au 31 décembre 2015 une étude rétrospective à visée descriptive a été réalisé dans les CAT d'Abidjan,les services de PPH d'Abidjan, à l'IPCI, au CEDRES. Les patients inclus étaient suivis pendant 9 mois. Des 132 patients éligibles et inclus, 34, 64, 14, 13 et 7 étaient respectivement des cas d'échec de primo-traitement, d'échec de traitement, de rechute de primo-traitement, de rechute de retraitement et de divers autres cas. Les mutations détectées dans le gène rpoB étaient de 143. Les mutations au codon 526 étaient les plus fréquentes avec 37,76%. Des 132 inclus, 110 disposaient d'un cliché radiographique du thorax dont 105 avaient des lésions étendues. Le taux de conversion bacillaire après 2 mois de traitement était de 38,21%. Les patients en échec étaient 24 (18,18%) dont 14 en retraitement. L'Odd ratio chez les patients ayant des lésions pulmonaires étendues,des cas de TBMR type infection mixte avec au moins 3 mutations dans rpoB étaient respectivement de 0.94 IC95% [0,09-23,35], 2 IC95% [0,75-5,35]. Aucun facteurs prédispose à un échec.

Sputum from included patients was cultured monthly for 9 months. Out of 132 patients put on a short 9-month regimen, 22 (16.66%) were HIV infected including 6 cases of relapse and 4 lost to follow-up. At the consolidation phase of the short course of MDR-TB, cultures were negative for 14/18 (77.77%) and positive for 4/18 (22.22%). The D516V mutation in the rpoB gene was the most frequent among cured patients with 42.85%. One failure was observed in a patient infected with a double mutated strain (L511P, S522T). Among the failures, 2 patients were XDR. The estimated Od ratio according to rpoB gene mutation type and clinical category was 0.81 95% CI [0.13 ;20.57] and 0.27 95% CI [0.002 ; 2.82], respectively. The type of rpoB mutation and clinical category did not influence the treatment outcome of HIV/TBMR co-infected patients. 52 pp. Englisch.

De los 132 pacientes sometidos a un régimen corto de 9 meses, 22 (16,66%) estaban infectados por el VIH, incluidos 6 casos de recaída y 4 perdidos durante el seguimiento. En la fase de consolidación del curso corto de la TB-MDR, los cultivos fueron negativos en 14/18 (77,77%) y positivos en 4/18 (22,22%). La mutación D516V en el gen rpoB fue la más frecuente entre los pacientes curados, con un 42,85%. Se observó un fallo en un paciente infectado con una cepa con doble mutación (L511P, S522T). Entre los fracasos, 2 pacientes eran XDR. La Od ratio estimada por tipo de mutación rpoB y categoría clínica fue de 0,81 IC 95% [0,13; 20,57] y 0,27 IC 95% [0,002; 2,82] respectivamente.
