They impair bone mineralization and can also alter bone mineral density (BMD). Low BMD is a risk factor for osteoporosis and bone fracture, not only in later years, but also in childhood and adolescence. In our study we compared BMD, bone age and vitamin D levels within different groups of children. We also measured body composition and evaluated usefulness of bioelectrical impedance analysis (BIA) in predicting low BMD. Serologic markers EMA and t-TG were measured to determine the compliance with gluten-free diet. The results of the study show significant decrease in serum vitamin D levels in patients with celiac disease and IBD compared to the control group. Body composition, measured with BIA did not correlate with BMD so we cannot confirm the value of this method in diagnosis of low BMD. However, the correlation between BMD and t-TG values in celiac patients was confirmed. Therefore, the importance of strict gluten-free diet is emphasized. 60 pp. Englisch.…

Elles alterent la mineralisation osseuse et peuvent egalement modifier la densite minerale osseuse (DMO). Une faible DMO est un facteur de risque d'osteoporose et de fracture osseuse, non seulement a un age avance, mais egalement pendant l'enfance et l'adolescence. Dans notre etude, nous avons compare la DMO, l'age osseux et les taux de vitamine D au sein de differents groupes d'enfants. Nous avons egalement mesure la composition corporelle et evalue l'utilite de l'analyse d'impedance bioelectrique (AIB) pour predire une faible DMO. Les marqueurs serologiques EMA et t-TG ont ete mesures afin de determiner le respect du regime sans gluten. Les resultats de l'etude montrent une diminution significative des taux seriques de vitamine D chez les patients atteints de la maladie coeliaque et de MICI par rapport au groupe temoin. La composition corporelle, mesuree par BIA, n'a pas presente de correlation avec la DMO; nous ne pouvons donc pas confirmer l'utilite de cette methode dans le diagnostic d'une faible DMO. En revanche, la correlation entre la DMO et les valeurs de t-TG chez les patients coeliaques a ete confirmee. Par consequent, l'importance d'un regime strict sans gluten est soulignee. No nosso estudo, comparamos a DMO, a idade ossea e os niveis de vitamina D em diferentes grupos de criancas. Medimos tambem a composicao corporal e avaliamos a utilidade da analise de impedancia bioeletrica (BIA) na previsao de uma DMO baixa. Foram medidos os marcadores serologicos EMA e t-TG para determinar o cumprimento da dieta sem gluten. Os resultados do estudo revelam uma diminuicao significativa dos niveis sericos de vitamina D em doentes com doenca celiaca e DII, em comparacao com o grupo de controlo. A composicao corporal, medida atraves da BIA, nao apresentou correlacao com a DMO, pelo que nao podemos confirmar o valor deste metodo no diagnostico de baixa DMO. Nel nostro studio abbiamo confrontato la BMD, l'eta ossea e i livelli di vitamina D in diversi gruppi di bambini. Abbiamo inoltre misurato la composizione corporea e valutato l'utilita dell'analisi dell'impedenza bioelettrica (BIA) nella previsione di una bassa BMD. Sono stati misurati i marcatori sierologici EMA e t-TG per determinare l'aderenza alla dieta priva di glutine. I risultati dello studio mostrano una significativa diminuzione dei livelli sierici di vitamina D nei pazienti affetti da celiachia e da malattie infiammatorie croniche intestinali (MICI) rispetto al gruppo di controllo. La composizione corporea, misurata con la BIA, non ha mostrato alcuna correlazione con la BMD; pertanto, non e possibile confermare l'utilita di questo metodo nella diagnosi di una bassa BMD. Sie beeintraechtigen die Knochenmineralisierung und koennen zudem die Knochenmineraldichte (BMD) veraendern. Eine niedrige BMD ist ein Risikofaktor fuer Osteoporose und Knochenbrueche, nicht nur im hoeheren Alter, sondern auch im Kindes- und Jugendalter. In unserer Studie verglichen wir die BMD, das Knochenalter und die Vitamin-D-Spiegel bei verschiedenen Gruppen von Kindern. Zudem haben wir die Koerperzusammensetzung gemessen und die Aussagekraft der bioelektrischen Impedanzanalyse (BIA) bei der Vorhersage einer niedrigen BMD bewertet. Zur Bestimmung der Einhaltung einer glutenfreien Ernaehrung wurden die serologischen Marker EMA und t-TG gemessen. Die Ergebnisse der Studie zeigen einen signifikanten Rueckgang der Vitamin-D-Serumspiegel bei Patienten mit Zoeliakie und IBD im Vergleich zur Kontrollgruppe. Die mittels BIA gemessene Koerperzusammensetzung korrelierte nicht mit der BMD, sodass wir den Wert dieser Methode bei der Diagnose einer niedrigen BMD nicht bestaetigen koennen.
