Algunos han representado verdaderos avances en el campo, pero otros no han demostrado ser valiosos para el conocimiento con el que practicamos la cirugía de hombro ni para los pacientes a quienes intentamos ayudar a recuperar la comodidad y la función de su hombro. Es hora de preguntar: «¿El costo incrementado de esta nueva herramienta de diagnóstico o implante ha sido justificado por el mejoramiento en los resultados clínicos experimentados por el paciente?» Por ejemplo, ¿la mayor precisión en la medición de la retroversión glenoidea usando complejos programas de reconstrucción tridimensional se ha traducido en una mejoría demostrable en los resultados del paciente? Siguen surgiendo nuevos métodos para obtener imágenes y tratar de reparar los desgarros del manguito rotador, pero ¿estos métodos conducen a tasas más altas de integridad del manguito? La característica más interesante de la cirugía del hombro es su dinamismo: los paradigmas existentes son constantemente desafiados, cada día se están evaluando nuevos enfoques para el diagnóstico y el tratamiento. La evidencia publicada está siendo continuamente evaluada y reevaluada para extraer una orientación para nuestra práctica. Los cirujanos se apegan a estándares cada vez más altos.
