Despite its comparative frequency, however, the subject has received but scant mention in all textbooks upon ophthalmology, and there is a wide difference of opinion among authors regarding its causation. / Having but recently encountered some cases of this nature I determined to report these, together with a series of similar cases from the Wills Eye Hospital records, with a view to ascertaining what the causation of the delirium might be, and what were the best measures to adopt for its prevention and relief. / Disturbances of the mind after operations upon the eye usually follow the removal of cataract; they have, however, been occasionally observed after various other surgical procedures in ophthalmology. The mental derangement, as a rule, takes the form of a delirium which appears usually upon the evening of the third day after the operation and continues a few days to a week, according as the surgeon is prompt in combating it, and fortunate in his choice of remedies." G., female, sixty-two years of age, consulted the writer in April, 1903, on account of poor sight. She said that she had worn glasses since childhood, having inherited myopia, but that she had had satisfactory vision with her glasses until two years before she came to consultation. At that time both eyes became the seat of a rheumatic iritis, for which she was treated by a colleague. The inflammation was, how- ever, very intense, so that vision was much impaired after the acute symptoms subsided. Upon this account an iridectomy had been performed upon both eyes, but without avail, vision still remaining low. / When examined by the writer, both eyes were practically quiet, though the results of iridocyclitis were very evident, the irides being plastered down to the partially cataractous lenses. The attempts at the previous iridectomies were plainly visible, a small opening in the irides of both eyes about two mm. broad, representing the colobomata. Tension was full in each eye, and the sclera was rigid. Vision equalled 1-70 in the right eye and ½ /70 in the left. No view of the fundus was obtainable, on account of the opacity in the lenses.
