Eine Reihe von Pharmaka (insbesondere Hemmstoffe der Reversen Transkriptase) haben ihre klinische Wirksamkeit bewiesen, viele inte…ressante Substanzen mit sehr unterschiedlichen Angriffspunk ten im HIV-Vermehrungszykus (z.B. 'entry' Inhibitoren, Hemmstof fe der RNase H, usw) befinden sich in verschiedenen Stadien der präklinischen und klinischen Entwicklung. Leider sind diese neuen Substanzen noch nicht für die Therapie verfügbar. 2. Ferner wurde in den letzten Jahren zunehmend deutlich, dass durch die bisher klinisch angewendeten Hemmstoffe der Reversen Transkriptase (NRTI, NNRTI) im Allgemeinen bei fortgeschritte ner AIDS-Erkrankung nur eine partielle immunologische Rekonsti tution erreichtwerden kann. Hinzu kommt, dass die erreichten Ver besserungen für den Patienten lediglich vorübergehend zu einer Lebensverlängerung führen, aber leider nicht kurativ sind. 3. Offen ist, inwieweit durch Kombinationen von Hemmstoffen der Reversen Transkriptase (AZT plus DDC oder AlT und DDI) die immunologische Rekonstitution optimiert werden kann. Die ge nannten Kombinationen scheinen zu einer umfangreicheren und länger anhaltenden Verbesserung des immunologischen Status zu führen; sie sind der Monotherapie mit Hemmstoffen der Re versen Transkriptase überlegen. 4. Wie ausgeführt, wirkt bis heute keine der verfügbaren Therapien kurativ und die Forderung zur intensiven Pharmakon-Entwicklung ist dringender als je zuvor. Offenbar konzentriert sich die gegen wärtige HIV-Forschung zu sehr auf die Blockade der Infektion durch freie Viren und nicht darauf, die virusinfizierte Zelle direkt anzugreifen.
