Diese Tatsache wird durch…einen steten Anstieg der Anforderun gen an Halt und Funktion einer totalen Prothese seitens der Patienten noch mehr betont. Die chirurgische Hilfestellung durch den Kieferchirurgen war jahrzehntelang durch standardisierte, traditionelle chirurgische Techniken gepragt, die insgesamt jeweils nur geringen Modifikationen unterworfen wurden und deren Leistungsfahigkeit zumindest universitar nur selten in Frage gestellt wurde. Erst kritische Untersuchungen der letzten Jahre zeigten die oft ungunstige und fragwurdige Relation zwischen Aufwand, Patientenbelastung und Komplikationsgefahr einerseits und dem oft maf3i gen Erfolg und Vorteil fur den Patienten andererseits auf. Die daraus resultierenden Bestrebungen, die traditionellen Wege der praprothetischen Chirurgie zu verlassen oder zumindest in wesentlichen Punkten zu modifi zieren, wurden bestarkt durch die Einfiihrung und das Angebot der in einem hohen Grad vertraglichen und einsetzbaren Knochenersatzmaterialien, der verschiedenen Formen des Hydroxylapatits, und die Bekanntgabe und Einfiihrung von nunmehr wissenschaftlich in einem hohen Maf3 untermau erten enossalen Implantationsmethoden mit den in der Literatur in den letzten Jahren angegebenen guten Prognoseraten. Diese drei unterschiedli chen Behandlungsvarianten - die traditionellen, praprothetisch-chirurgi schen Verfahren, der Einsatz von Hydroxylapatit-Keramiken und die Implantation von enossalen Pfeilern aus Titan, Tantal oder Kerami- standen in den letzten Jahren zur Erreichung ein und desselben Zieles in Konkurrenz zueinander, namlich, den zahnlosen Patienten mit Hilfe prothetischer Maf3nahmen wieder zu einem heutigen modernen Anforde rungen entsprechenden funktionellen Zahnersatz zu verhelfen.
