Sie haben auBerdem die Aufgabe, den prokaryontischen Wirtskeimen die Fahigkeit zu vermitteln, ihre durch Mutationen unbrauchtJar geworde ne Erbsubstanz durch funktionsfahige zu ersetzen. Den Eukaryonten stehen hierzu die sexuellen Rekombinationsmbglichkeiten zur Verfu gung. Die R-Faktoren haben deshalb uber die Vermittlung von Resi stenzeigenschaften hinaus EinfluB auf die Entwicklung nosokomialer Infektionen. Da ihre Verbreitung bereits ein groBes AusmaB ange nom men hat, sollte ihre weitere Selektion bei Krankheitserregern ein geschrankt werden. Neben der Vermeidung unnbtiger Antibiotikaga ben in der Klinik und in auBermedizinischen Bereichen sollten solche Antibiotika bevorzugt werden, die R-Plasmide nicht selektieren. Hier zu gehbrt Fosfomycin, welches trotz intensiver Verwendung bislang noch keine plasmidische Resistenzentwicklung zu erkennen gab. Auch eigene Untersuchungen sprechen dafUr, daB noch keine plas midische Fosfomycin-Resistenz aufgetreten ist.
