One of the most challenging areas in reconstructive surgery to emerge in the last decade has been microneurovascular surgery. The experimental work described and fully illustrated in this book provides a sound technical basis for the application of these surgical techniques. It also contains detailed information on the precise instrumentation required and an explanation of the pathophysiology of microvascular occlusion. These techniques whilst they have originated in the field of reconstructive surgery, will become increasingly important in all branches of surgery. The techniques of repair and grafting of small arteries and veins of approximately 1mm in diameter are described, incorporating a detailed study of the histopathology of these procedures. The important area of the organisation of a microsurgery unit at both laboratory and clinical levels with maximum community service is stressed. An up-to-date series of chapters on clinical microvascular surgery forms the main section of the book. These include a wide coverage of digital and major replantation surgery. The indications, techniques, postoperative management and early and long term results are discussed. Composite tissue transfers involving microvascular free flaps and omental transfers with their wide range of application are dealt with, as is the single stage double toe transfer for severely disabled hands. Chapters on microneural repairs and grafts add further to the book, with a final chapter on the microsurgery of miscellaneous structures and small organs including the fallopian tube, vas deferens, colonic and jejunal transfer, undescended testes, experimental renal transplants and penile replantation.

With the introduction of the operating microscope by Nylen (1921), fine detailed anatomy was made visible, so that structures could be prepared, pre sented and manipulated. Nylen used the microscope for treating dis eases of the middle ear. In 1946 Perrit in the U. S. A. introduced the operating microscope into the field of ophthalmic surgery. In 1964, Smith, Kunze and Mitchen published reports on their clinical experi ence using the operating microscope in nerve surgery. Using this tech nique, it was possible to divide a peripheral nerve into individual fascicle bundles and reapir each bundle precisely. Following on from these developments, the term' microsurgery' was introduced to cover that branch of surgery which was carried out using the' equiped' eye - either by using 10upe spectacle magnifica tion or, more commonly, the operating microscope. The term 'micro vascular surgery' developed for the operative treatment of small ves sels below 2 mm in diameter. Experimental studies for the use of microsurgery in the clinical situation developed alongside its use on small research animals for transplantation models in immunological study. Microsurgical techniques were used in the transplantation of rat liver, kidneys, pancreas, heart and other organs. The clinical appli cation of these microsurgical techniques is nowadays called 'recon structive microvascular surgery'.
