At the outbreak of the Civil War, Dr. Smith was appointed the surgeon general of Pennsylvania, originated the plan for the removal of the wounded from the battlefield to large hospitals in major cities, organized a corps of surgeons on steamers that were used as floating hospitals at the siege of Yorktown and tended to the wounded after various battles, i.e., Cold Harbor and Antietam.
