Das Operationsrisiko konnte in den letzten Jahren durch Ausbau und Stan dardisierung der rekonstruktiven Gefal3eingriffe drastisch gesenkt werden. 2. Flir die Operationsindikation sind die Anzahl der erkrankten hirnversorgenden Arterien und deren Gefal3morphologie, d. h. Nachweis eines hohen hiimodyna mischen oder embolischen Risikos, das Computertomogramm des Schiidels und der Ausschlul3 gravierender internistischer Begleiterkrankungen von ausschlag gebender Bedeutung. Der hiiufigste limitierende Faktor stellt eine klinisch rele vante Miterkrankung der Koronargefal3e dar (Hiiufigkeit ca. 60%). Daher gilt es, vor der Planung eines prophylaktischen Eingriffs das koronare Risiko genau ab zuschiitzen und ggf. einen simultanen oder zeitlich versetzten Kombinationsein griff an den Koronararterien und an der Karotis vorzunehmen. 3. Die operativen Behandlungserfolge stehen und fallen mit der Verfl. igbarkeit eines in der Karotischirurgie erfahrenen Operationsteams. Nach wie vor gilt das von J.
