L'Ordine e Sacra Religione di San Lazzaro sorse in Terrasanta, probabilmente prima o contemporaneamente alle crociate, e oltre alla funzione militare e religiosa ebbe anche quella di curare, con conoscenze e mezzi del tempo, la terribile malattia della l…ebbra. I cavalieri lazzariti avevano la duplice veste di poveri lebbrosi destinati a una morte lenta e dolorosa, al pari di coloro che curavano, e temuti guerrieri che non temevano la morte, disposti a morire in battaglia, sinché il male concedeva loro di portare le armi. Come altri ordini militari e religiosi, specie dopo la perdita degli ultimi avamposti in Oriente, anche quello lazzarita estese la propria presenza fino alla Sardegna, con il principale compito di curare il male quando coloro che avevano partecipato alle crociate ei pellegrini che ritornavano dai Luoghi Santi . Tuttavia le condizioni giuridiche-istituzionali dell'Isola erano differenti da quelle esistenti sul Continente: nel medioevo, infatti, esistevano quattro Stati nazionali con propri istituti e retti da un sovrano non necessariamente ereditario (giudice), e fra questi era sconosciuto l'istituto del feudo, di derivazione germanica. La peculiare conformazione istituzionale di questi stati, se era certamente ostativa per una presenza "politica" di taluni Ordini militari, come quello del Tempio o dei cavalieri di San Giovanni (di Rodi, di Malta), non lo era per un Ordine che aveva come l'ultimo rimasto lo scopo né politico né militare, e perciò non sospetto, della cura della lebbra, con conseguente fondazione di lebbrosari. I lebbrosi erano considerati, nel medioevo, maledetti da Dio e non era presente alcuna autorità, né civile né religiosa, che avesse come ragion d'essere di soddisfare i loro bisogni materiali. L'esistenza di tali ospedali specialistici, perciò, poteva essere percepita dai governanti sardi come un valido supporto per le esigenze delle esperienze isolane fra cui, ben presto, si era esteso il contagio. E infatti quando si alterarono tali condizioni di equilibrio instabile di politica internazionale, con il consolidarsi del potere esclusivo di una di esse, iniziò un inarrestabile declino della incidenza della presenza degli stabilimenti di alcuni Ordini.
