La Pubalgia rappresenta una delle sindromi dolorose più trattate dai fisioterapisti, ne soffrono moltissimi sportivi, ma anche chi soffre di squilibri muscolari, patologie muscolari e tendinee, patologie infettive e tumorali.
Se la sintomatologia dolorosa viene sottovalutata, ignorata, trattata con sintomatici senza comprenderne le cause né curarla può cronicizzare, cioè essere memorizzata a livello della corteccia cerebrale, ed accadere che il dolore sia evocato ancor prima che l’azione scatenante si compia. Questo meccanismo ha un valore protettivo, impedirci l’azione che provoca ulteriore dolore, ma di fatto provoca un’espansione dell’area dolorosa, e limita l’attività sportiva per esempio, che invece potrebbe, grazie alle endorfine liberate antagonizzare il dolore stesso. È un cane che si morde la coda.
I casi di pubalgia, secondo le rilevazioni dei verbali di pronto soccorso, negli ultimi 10 anni sono aumentati dal 5 al 15 % di tutti gli infortuni sportivi.
Nella maggior parte dei casi si tratta di eventi flogogeni a carico delle inserzioni tendinee causate dall’abuso o dall’uso improprio. L’infiammazione riguarda le inserzioni dei muscoli adduttori della coscia, ma anche retto addominale, pettineo, piramidale, retti e obliqui addominali, trasverso addominale, dovuta a sovraccarico funzionale. Molti di questi muscoli presentano inserzione sulla sinfisi pubica, come gli adduttori della coscia e il retto addominale.
Il dolore è riferito all’inguine, può irradiarsi all’addome e interno coscia. Tipicamente compare al risveglio, aumenta nei primi minuti di movimento e regredisce con il riscaldamento.
Il dolore pubico si ha soprattutto quando si usano gli adduttori, cioè quando si eseguono movimenti istantanei, come alzarsi dalla sedia, fare pochi passi, calciare una palla.
La muscolatura adduttoria appare contratta e la pressione sul pube è dolorosa.
La prima causa di dolore è l’infiammazione dovuta a microtraumi ripetuti e microlesioni dei tendini (mio-entesiti), per movimenti improvvisi di torsione.
Al secondo posto, dopo gli sportivi, ci sono le giovani donne, in particolare sono le donne in gravidanza a soffrire di dolori a carico del pube. Infatti la sinfisi pubica è interessata nelle modificazioni che avvengono durante la gravidanza in preparazione al parto. Si potrebbe chiamare, secondo alcuni, sindrome retto-adduttoria, se il retto si indebolisce l’adduttore «tira di più».
Un fattore che predispone alla pubalgia è la posizione da seduti con la caviglia sopra al ginocchio opposto, cioè in massima rotazione di anca: in questi casi di solito è sufficiente consigliare riposo, crioterapia, e se persiste una consulenza ortopedica, che potrà evidenziare o meno un danno organico all’articolazione coxofemorale. Nella forma cronica ogni movimento che riguarda l’anca o la flessione del tronco produce dolore, che può irradiarsi anche al testicolo.
Sono state identificate finora 72 cause di pubalgia.

Right of withdrawal for consumers
(A consumer is any natural person who enters into a legal transaction for purposes that are predominantly neither commercial nor self-employed).
Right of withdrawal
If you are a consumer you can withdraw from the contract in accordance with the following. Consumer means any natural person who is acting for purposes which are outside his trade, business, craft or profession.

Statutory right to withdraw
You have the right to withdraw from this contract within 14 days without giving any reason.
The withdrawal period will expire after 14 days from the day on which you acquire, or a third party other than the carrier and indicated by you acquires, physical possession of the last good or the last lot or piece.
To exercise the right of withdrawal, electronically fill in and submit a clear statement on our website, under "My Purchases" in "My Account". We will communicate to you an acknowledgement of receipt of such a withdrawal on a durable medium (e.g. by e-mail) without delay.
To meet the withdrawal deadline, it is sufficient for you to send your communication concerning your exercise of the right of withdrawal before the withdrawal period has expired.
Effects of withdrawal
If you withdraw from this contract, we will reimburse to you all payments received from you, including the costs of delivery (except for the supplementary costs arising if you chose a type of delivery other than the least expensive type of standard delivery offered by us).
We may make a deduction from the reimbursement for loss in value of any goods supplied, if the loss is the result of unnecessary handling by you.
We will make the reimbursement without undue delay, and not later than 14 days after the day on which we are informed about your decision to withdraw from this contract.
We will make the reimbursement using the same means of payment as you used for the initial transaction, unless you have expressly agreed otherwise; in any event, you will not incur any fees as a result of such reimbursement.
We may withhold reimbursement until we have received the goods back, or you have supplied evidence of having sent back the goods, whichever is the earliest.
You shall send back the goods or hand them over to Buchpark, Trebbin, Germany, without undue delay and in any event not later than 14 days from the day on which you communicate your withdrawal from this contract to us. The deadline is met if you send back the goods before the period of 14 days has expired. You will have to bear the direct cost of returning the goods. You are only liable for any diminished value of the goods resulting from the handling other than what is necessary to establish the nature, characteristics and functioning of the goods.
Exceptions to the right of withdrawal
The right of withdrawal does not apply to:
- The delivery of newspapers, journals or magazines with the exception of subscription contracts; and
- The supply of digital content which is not supplied on a tangible medium (e.g. on a CD or DVD) if you accepted when you placed your order that we could start to deliver it, and that you could not withdraw once delivery had started.
