Ces décorations (ou mutilations ?) sont souvent associées à des rites d'initiation qui distinguent les adultes fertiles des enfants prépubères, assurant ainsi une identité sociale stable. De telles pratiques sont toujours observées aujourd'hui dans l'ensemble de l'Afrique. Dans certains cas, les opérations de ce genre, comme la circoncision, suggèrent le passage d'un monde biologique, humain, à un monde plus symbolique. C'est clairement le cas en ce qui concerne les religions juive et islamique. Mais différentes théories expliquent l'origine de la circoncision masculine. Selon l'une d'elles, la circoncision a d'abord été un moyen de purifier les individus et de diminuer leur plaisir sexuel. De Meo avance que la circoncision est apparue dans les régions désertiques de l'Afrique du Nord et du Proche-Orient, d'où elle s'est répandue vers l'Afrique subsaharienne, l'Océanie et peut-être vers certaines régions des Amériques (la civilisation maya, en effet, avait elle aussi un rite dans lequel les organes génitaux masculins étaient incisés à l'aide de feuilles d'agave). Dans un autre registre, l'introduction de la castration et la création des eunuques permet à un nombre limité d'hommes virils d'exploiter le système du harem et de garantir la pureté de leur propre ligne. Finalement, la castration (hormonale avec médication et sans opération chirurgicale) est utilisée dans le monde médical comme traitement de nombreuses maladies de la prostate.
