The dynamics that have been allowed to develop within the behemoth that the NHS has become, are destroying it.
The basics of health care as in the early years of the NHS need to be returned to when clinicians were listened to and if funding allowed, changes were dealt with by administrators.
With the arrival of the ‘age of management’ towards the end of the last century, the power of change was gradually taken over by management and clinicians felt no longer listened to.
Systems and the bottom line, finance, had taken over.
From 1997 to 2012 health expenditure in the NHS increased by 373%..This massive extra funding did not result in a proportionate health gain.
Local management now has to conform to the edicts from the Department of Health resulting in the top down NHS we now have, which as we saw during Covid, was found wanting.
Tyranny of a System - The NHS, is a history of the changes in the NHS which have over the years resulted in a system where highly qualified and trained staff don’t necessarily, unlike their predecessors, feel proud of where they are working.
To many they see themselves as working in a rules-based way where clinical freedom and judgement doesn’t count as it did in the past.
There is no question that the NHS needs radical reform as it is bankrupting the UK.
It needs to go back to again becoming a bottom-up organisation. In addition many of the changes itemised here need to be reversed. As NHS disasters occur, eg midwifery ones, it is obvious to me that the teamwork, Obstetrician/midwife of before, needs restoring.
The current crisis in General Practice was predictable as in my chapters, ‘QOF and Fools Gold’
As regards - 'more funding is needed' - in my view, this would be -'throwing good money after bad'.
My conclusion is that our health service is going to hell in a hand cart, the irony being that the people who have been pushing it all think they are doing it for the right reasons, while simultaneously sowing the seeds of the destruction of our nation’s treasure, the NHS.
The idea of a Royal Commission to ‘sort the NHS’ won’t work, as such an exercise will inevitably entail having to be advised by the folk already pushing the handcart.
