Add to basketPaperback. Most Americans have health insurance. Almost none of them understand it. A national survey found that only 4% of American adults could correctly define all four basic health insurance terms: deductible, premium, copay, and coinsurance. If you have ever opened a medical bill and genuinely not understood why you owed what it said - you are not alone, and it is not your fault. Nobody explained this to you. This book does. The Fine Print Is Costing You is a plain-English guide to understanding, using, and fighting back within the American health insurance system - written by someone who spent fifteen years inside a health insurance company processing claims and reviewing appeals. Laura Stratton knows where the confusion comes from, where the money goes, and exactly what policyholders need to know that no one ever tells them. What this book covers: Health insurance basics explained clearly - how premiums, deductibles, copays, coinsurance, and out-of-pocket maximums work and interact How to choose between HMO, PPO, EPO, and HDHP plans at open enrollment The ACA Marketplace, subsidies, and how to qualify How claims are processed - what happens between your doctor's visit and your bill How to read an Explanation of Benefits (EOB) field by field Prior authorization - what it is, when it applies, and what to do when it is denied Prescription drug coverage, formularies, and lower-cost alternatives HSAs and FSAs - how they work and how to avoid losing money Mental health coverage and your rights under federal parity law Surprise medical bills and the No Surprises Act How to identify and dispute medical billing errors How to appeal a denied health insurance claim - step by step Your rights to an independent external review when the insurer says no twice Includes an Open Enrollment Plan Comparison Guide, EOB Field Reference, Sample Denial Appeal Letter, and a complete Glossary of Health Insurance Terms in Plain English.
