"The extraordinary variability of the pictures that denote pituitary dysfunction bespeaks the many functions that must be allotted to this small, complex structure. Although much has been learned from direct observation in the clinic and from the contributory evidence of the experimental laboratory, one still cannot speak too dogmatically concerning what is and what is not of direct pituitary origin. Some of the symptoms in the subject with pituitary dysfunction can be explained only on the basis of an interrelationship between the pituitary and other organs of the endocrine chain. Thus the lowered metabolic rate and the accession of fat of the Fröhlich syndrome are not directly of pituitary origin, but rather an interaction between the pituitary and the thyroid, an expression of hypothyroidism. There is still confusion concerning the part the tuber cinereum plays in the picture once exclusively ascribed to pituitary malfunction." Frazier "Frazier held Cushing's erroneous view about the pituitary origin of diabetes insipidus (hyperfunction of the posterior lobe) and Frohlich's syndrome (decreased secretion of the posterior lobe into the third ventricle). It was not until the late 1920s that Frazier took into account Bailey's theory about the involvement of the tuber cinereum in Frohlich's syndrome. Frazier, however, was a pioneer in recognizing cachexia as a typical symptom of third ventricle CPs. He noticed that the pituitary gland was intact in the 3 CP brain specimens he reviewed in his article titled "Pituitary cachexia." Two of these cases, one published by Lang and the other by Hönlinger, as well as the CP case with cachexia in Frazier's own series (case 52), actually correspond to strictly intraventricular CPs. These findings probably led Frazier to dismiss Simmonds' prevailing pituitary theory in favor of Aschner/Erdheim's hypothalamic theories." Neurosurg 133 (2020): pp.1739-1752 Charles Harrison Frazier (1870-1936) was a visionary neurosurgeon of his time. He devoted himself almost exclusively to neurosurgery based on his training in Germany on neurology, surgery, and surgical pathology and his accomplishments as a military physician during war years.
