As National Medical Director of TLC The Laser Eye Centers, the largest laser vision provider in North America, Louis E. Probst, MD has performed over 100,000 LASIK procedures over 16 years; has written 5 reference textbooks on refractive surgery and LASIK, over 80 book chapters on ophthalmology and LASIK, and over 50 articles in peer-reviewed publications; and has designed 10 instruments for LASIK. Dr. Probst has given hundreds of lectures at national and international level meetings on his various innovations and insights into improving patient outcomes in ophthalmology and refractive surgery.
Dr. Probst has been an innovator in many refractive surgery techniques that include the treatment of LASIK flap striae, cross cylinder ablation techniques for the treatment of high mixed astigmatism, bioptics for the correction of high hyperopic astigmatism, single-use instrumentation for LASIK, flap/zap femtosecond LASIK, custom zone CustomVue LASIK, and customized oval flap LASIK techniques.
Dr. Probst serves as the chief surgeon at several TLC laser center locations in Illinois, Wisconsin, South Carolina, and Florida and is a member of the Clinical Advisory Group of TLC to which provides guidance and recommendation for over 500 ophthalmologists and 5000 optometrists who work within the TLC system. Dr. Probst also serves as a trainer and educator at TLC for surgeons interested in learning about the latest techniques and technology. Dr. Probst also trains residents and fellows in ophthalmology as a volunteer assistant professor at the University of Wisconsin.
Julie H. Tsai, MD is an Assistant Professor of Ophthalmology and is heavily involved in resident training. She has served as the Residency Program Director for 2 training programs and continues to promote ophthalmic education and mentorship through her work in various committees and organizations on both local and national levels. She has written several chapters and peer-reviewed publications and has been an invited lecturer on topics involving anterior segment reconstruction, cornea, and uveitis.
George Goodman, OD currently serves as the Senior Manager of Clinical Affairs for Refocus Group, Inc, in Dallas, Texas. Prior to working for a medical device company, Dr. Goodman served as the Clinical Director at TLC The Laser Eye Centers in Chicago where he worked for the National Medical Director, Dr. Louis E. Probst. Before relocating to Chicago, Dr. Goodman was the Co-Founder and Clinical Director of Advanced Laser Center of Oklahoma in his hometown of Oklahoma City, Oklahoma. He began his career by working with his father and grandfather in private practice.
Dr. Goodman has the following associations: Board Member of the Optometric Council on Refractive Technology (OCRT); Co-Founder of the Optometric Education Council of America (OEC); Speaker―Inspire, Allergan, Alcon, Zeiss; Adjunct Faculty―Northeastern State University Oklahoma College of Optometry; Member―American Optometric Association (AOA), Oklahoma Association of Optometric Physicians (OAOP), and Illinois Optometric Association (IOA). Dr. Goodman previously served on the Board of Directors and the Medical Advisory Board for the Oklahoma Lupus Association. Dr. Goodman has written and published 3 books. He also served as the author and executive editor of “Focus on Co-Management,”TLC The Laser Eye Center’s monthly affiliate newsletter.

Statutory Right to cancel
You have the right to cancel this contract within 14 days without giving any reason.
The cancellation period will expire after 14 days from the day on which you acquire, or a third party other than the carrier and indicated by you acquires, physical possession of the the last good or the last lot or piece.
To exercise the right to cancel, you must inform us, HPB-Red, 5803 E. Northwest Hwy., 75231, Dallas, Texas, U.S.A., +1 214-819-9556, of your decision to cancel this contract by a clear statement (e.g. a letter sent by post, fax or e-mail). You may use the attached model cancellation form, but it is not obligatory. You can also electronically fill in and submit a clear statement on our website, under "My Purchases" in "My Account". If you use this option, we will communicate to you an acknowledgement of receipt of such a cancellation on a durable medium (e.g. by e-mail) without delay.
To meet the cancellation deadline, it is sufficient for you to send your communication concerning your exercise of the right to cancel before the cancellation period has expired.
Effects of cancellation
If you cancel this contract, we will reimburse to you all payments received from you, including the costs of delivery (except for the supplementary costs arising if you chose a type of delivery other than the least expensive type of standard delivery offered by us).
We may make a deduction from the reimbursement for loss in value of any goods supplied, if the loss is the result of unnecessary handling by you.
We will make the reimbursement without undue delay, and not later than 14 days after the day on which we are informed about your decision to cancel with contract.
We will make the reimbursement using the same means of payment as you used for the initial transaction, unless you have expressly agreed otherwise; in any event, you will not incur any fees as a result of such reimbursement.
We may withhold reimbursement until we have received the goods back or you have supplied evidence of having sent back the goods, whichever is the earliest.
You shall send back the goods or hand them over to us or HPB-Red, 3860 La Reunion Pkwy., 75212, Dallas, Texas, U.S.A., +1 214-819-9556, without undue delay and in any event not later than 14 days from the day on which you communicate your cancellation from this contract to us. The deadline is met if you send back the goods before the period of 14 days has expired. You will have to bear the direct cost of returning the goods. You are only liable for any diminished value of the goods resulting from the handling other than what is necessary to establish the nature, characteristics and functioning of the goods.
