Paperback. Safety is not achieved through procedures alone. It is sustained through effective systems, competent people, strong leadership, and a culture of continuous learning.Occupational Health and Safety Handbook - Volume 3: Human Factors, Competence, Culture, and Organisational Change provides a comprehensive and practical examination of the human and organisational factors that influence workplace safety performance.Moving beyond physical hazards and regulatory compliance, this volume explores how human behaviour, decision-making, competence, leadership, organisational culture, and workplace conditions interact to shape safety outcomes. It explains why serious incidents often arise from combinations of system weaknesses rather than isolated individual errors.Drawing on established human factors principles, organisational behaviour, and contemporary safety management approaches, the book provides a structured framework for understanding human performance, strengthening organisational capability, and developing resilient safety systems.Key Topics CoveredHuman Factors and Safety Performance: Human capabilities, limitations, cognitive processes, and their influence on workplace safety.Human Error and Systemic Failures: Understanding error mechanisms, underlying organisational conditions, and approaches to preventing recurrence.Decision-Making and Cognitive Bias: How workload, uncertainty, time pressure, and judgement influence operational decisions.Competence and Capability Assurance: Developing effective training, assessment, supervision, and professional development systems.Communication and Worker Engagement: Strengthening information sharing, consultation, participation, and operational coordination.Safety Culture and Behavioural Safety: Understanding organisational values, safety climate, behavioural influences, and sustainable improvement.Leadership and Organisational Accountability: Examining how leadership decisions, management practices, and organisational priorities affect safety performance.Organisational Change and Risk Management: Identifying and controlling safety risks arising from restructuring, technological changes, and evolving work arrangements.Organisational Learning and Resilience: Strengthening incident learning, organisational memory, adaptability, and continuous improvement.Psychological Health and Psychosocial Risk: Addressing work-related stressors, psychosocial hazards, and organisational approaches to worker wellbeing.Who Should Read This Book? This volume is designed for occupational health and safety professionals, HSE managers, engineers, supervisors, organisational leaders, consultants, trainers, researchers, and postgraduate students seeking a deeper understanding of the human dimensions of workplace safety. It is particularly valuable for professionals responsible for developing competence frameworks, improving safety culture, managing organisational change, strengthening leadership accountability, and investigating the underlying causes of workplace incidents. From Individual Error to Systemic UnderstandingRather than focusing solely on who made a mistake, this book encourages readers to examine how workplace conditions, organisational decisions, and management systems influence human performance.By integrating human factors, competence assurance, leadership, and organisational learning, Volume 3 provides practical insights for reducing preventable errors, strengthening workforce capability, and improving safety performance across complex and high-risk industries.A valuable professional reference for building sa Shipping may be from our UK warehouse or from our Australian or US warehouses, depending on stock availability.…

It is sustained through people, systems, and leadership.
In Volume 3 of the Occupational Health and Safety Handbook, the focus moves beyond physical hazards and regulatory compliance to examine the deeper drivers of occupational health, safety, and environmental (OHSE) performance — human cognition, competence, culture, and organisational design.
Across industries, serious incidents rarely result from a single unsafe act. They emerge from interacting system conditions: flawed decision environments, degraded competence frameworks, communication breakdowns, cultural drift, leadership failures, unmanaged change, and psychosocial pressures. Understanding these dynamics is essential for building resilient, high-performing organisations.
This volume provides a comprehensive, systems-based examination of the human dimensions of safety. It integrates contemporary safety science, human factors theory, organisational behaviour, and practical OHSE management principles into a coherent framework for modern risk management.
Inside this volume, you will explore:
• Human factors and the cognitive foundations of safety performance
• Human error versus systemic human failure
• Decision-making under pressure and cognitive bias
• Competence frameworks and capability assurance systems
• Information, instruction, supervision, and communication in practice
• Worker consultation and engagement structures
• Safety culture and safety climate development
• Behavioural safety and behaviour change strategies
• Leadership influence through a human factors lens
• Organisational change and risk integration
• Organisational learning, resilience, and memory
• Psychological health and psychosocial risk management
Written for safety professionals, organisational leaders, engineers, supervisors, and postgraduate students, Volume 3 bridges rigorous theory with practical application. It challenges traditional blame-based approaches and reframes safety as a product of system design, ethical leadership, and deliberate capability development.
Rather than asking, “Who failed?” this volume asks, “How did the system shape performance?”
By shifting focus from individual fault to systemic understanding, this handbook equips organisations to reduce error, strengthen competence, improve culture, and build sustainable safety performance in complex and high-risk environments.
Safety is not an outcome of compliance.
It is an outcome of design, leadership, and learning.
Volume 3 provides the roadmap.
