The Decisions You Make in the First Hour of Admission Shape Everything That Follows
Inpatient medicine moves fast. A patient arrives with overlapping problems, the chart is incomplete, consults are pending, and the plan you set today decides whether they go home safely in three days or return in seven. Hesitation at the bedside has a cost, and so does a gap in knowledge that you only discover in the middle of a shift.
This textbook was written to close that gap. It gives residents, nurse practitioners, physician assistants, and practicing hospitalists a structured, decision-focused guide to the full arc of inpatient care, from the first assessment through discharge.
What's Inside:
Admission and Initial Assessment: Organizing a focused workup, recognizing early warning signs, and setting priorities when time is short.
Clinical Reasoning: Frameworks for building a differential, testing it against the evidence, and avoiding the cognitive traps that lead to diagnostic error.
Common Inpatient Conditions: Cardiovascular, pulmonary, infectious, renal, gastrointestinal, endocrine, and neurological problems that fill a daily census.
Medical Consultation and Co-Management: Working alongside surgical and specialty teams and managing patients with several active conditions at once.
Safe Prescribing in the Hospital: Medication review, interactions, and high-risk situations that call for extra caution.
Transitions of Care and Discharge Planning: Preparing patients for a safe return home, reducing preventable readmissions, and communicating clearly with outpatient clinicians.
Quality, Safety, and Systems of Care: Practical tools for improving care on the floor, not just in a committee room.
Why Waiting Costs More Than You Think
Every shift without a dependable reference is a shift spent relying on memory alone. Small gaps compound: a missed red flag, a discharge plan that overlooks a medication change, a differential that closed too early. The clinicians who work with the most confidence aren't guessing well. They have the right guidance within reach when the moment arrives. Those who build that foundation early carry it through every rotation and every year of practice that follows.
What Sets This Textbook Apart:
- Decision-Focused Organization: Content is arranged around the choices you actually face at the bedside, not around theory for its own sake.
- Built for the Realities of the Ward: Written for busy clinicians who need clear answers between admissions, rounds, and handoffs.
- Full-Journey Coverage: One resource follows the patient from admission to discharge and the transition afterward.
- Frameworks You Can Reuse: Structured approaches that transfer from one case to the next, so your reasoning gets faster with every patient.
- Beyond the Clinical: Quality improvement, patient safety, and communication skills are treated as core parts of good inpatient care.
Perfect For:
- Internal medicine and family medicine residents on inpatient rotations
- Hospitalists building a dependable daily reference
- Nurse practitioners and physician assistants in inpatient settings
- Medical students preparing for clerkships and board examinations
This textbook is an educational and reference resource. It does not replace institutional protocols, current clinical guidelines, or individual clinical judgment.
Every case you manage without a trusted reference is a case you navigate without your strongest tool. Add this textbook to your professional library today and walk into your next shift prepared.
