Admission is only the beginning of hospital care.
The first hours establish the working diagnosis and immediate priorities. The next phase tests those assumptions against new laboratory results, imaging, treatment response, and changes in physiology. Later decisions determine whether therapy should continue, intensify, narrow, or stop. Eventually, attention shifts again—to recovery, medication reconciliation, functional readiness, discharge planning, and what must happen after the patient leaves the ward.
Hospital Medicine Principles For Practice follows that entire progression.
At first assessment, the emphasis is on building an accurate clinical picture from information that may still be incomplete. Presenting symptoms, comorbidities, medications, examination findings, diagnostic studies, and immediate safety concerns must be organized into priorities that can guide the opening plan.
During treatment, hospital medicine becomes an exercise in continuous revision. A reasonable plan at admission may no longer fit twelve hours later. New findings can narrow the diagnosis; organ function can change medication choices; complications can alter the level of monitoring required. The text therefore treats reassessment as part of management itself rather than something that occurs after treatment is finished.
As the hospital course evolves, attention extends beyond whether a disease is improving. Clinicians must consider complications of hospitalization, changing functional status, medication burden, consultation findings, response to therapy, risk of deterioration, and whether the patient is actually moving toward a safe next stage of care.
Recovery and transition complete the clinical episode. A discharge decision is not simply an administrative endpoint. It requires reconciliation of medications and unresolved problems, appropriate follow-up, clear communication, realistic recovery expectations, and transfer of responsibility without losing essential clinical information.
Across these stages, the book connects diagnostic judgment, treatment, surveillance, safety, recovery, and care coordination within one continuous framework. It is designed for physicians, residents, medical trainees, and other healthcare professionals who need to understand inpatient medicine as an evolving process rather than a sequence of disconnected tasks.
Its governing principle is continuity of judgment: every hospital decision should account for where the patient has been, what has changed, what requires attention now, and what must be safely carried forward into the next phase of care.
