When an exposure happens, every minute and every handoff matters
A needlestick at 2:00 a.m. is not only a clinical event. It is a time-critical coordination test involving first aid, exposure classification, source-patient testing, consent, HIV post-exposure prophylaxis access, hepatitis B status, hepatitis C follow-up, confidentiality, and a clean transfer of responsibility. When those steps live in separate policies or in staff memory, delays and documentation gaps multiply. This manual brings the full response into one governed operating system.
The problem this manual solves
The hardest failures rarely come from a total absence of policy. They come from unclear ownership, inconsistent triage, missing source information, an after-hours pharmacy gap, an undocumented callback, or a case that never reaches formal closure. The manual shows teams how to define the response clock, assign decision rights, separate clinical judgment from operational routing, and make every transfer visible enough to audit later.
A working system, not a loose collection of tips
The method follows one exposure from the first fifteen minutes through final follow-up. Route-and-fluid triage determines the next action; source testing and consent move on a parallel track; HIV, HBV, and HCV pathways remain distinct; and a single case record ties the work together. The result is a system staff can rehearse, leaders can measure, and clinicians can adapt to current guidance and local law.
Follow one connected implementation path
You will build the program charter and RACI, sort exposures by route, fluid, injury, and circumstance, coordinate source-patient testing, route HIV risk and PEP access, verify HBV immunity, schedule HCV follow-up, manage pregnancy, lactation, delayed reports, unknown sources, agency staff, and multi-site after-hours coverage, then close the loop with documentation, billing, training, mock drills, and KPI review.
Tools you can use immediately
Completed reference tools include a first-15-minute algorithm, route-and-fluid triage tree, source-testing workflow, HIV decision route, confidential HCV follow-up calendar, multi-site handoff flow, governance matrices, rapid-testing responsibility maps, call scripts, audit checks, and dashboard measures. They are designed as implementation examples rather than empty pages, so readers can see what a functioning program looks like before adapting it.
Built for the point of work
Use the manual in three modes: as a build guide for a new or repaired exposure program, as an urgent desk reference during a live incident, and as an audit framework after the event. The chapter sequence keeps the clinical pathways separate while preserving one accountable case owner. That structure helps prevent a result, referral, medication decision, or follow-up date from disappearing between departments.
Evidence, judgment, and responsible use
This is operational and educational guidance, not individualized medical or legal advice. PEP regimens, laboratory practice, consent law, employer obligations, and public-health guidance change and vary by jurisdiction. Every organization must verify current requirements with qualified clinicians, counsel, its compliance office, and the cited authorities before using a threshold, regimen, or timeline in a real case.
Make the next decision easier to see
Move from scattered policy fragments and improvised handoffs to a rehearsed, traceable response that protects the exposed worker and gives leaders a defensible record of what happened. Open the manual before the next event, assign the roles, test the after-hours route, and make reliable case closure part of the program rather than an afterthought.
