41pp extract, printed in double columns, illustrated with 22 photographs including portraits of the author, John Hancock Douglas, Henry B. Sands, and Fordyce Barker, who were the three other attending physicians, several of Grant at various ages, Grant's parents, two group portraits of the Grant family, and several of notes Grant wrote to the author during his final days, salvaged from damaged issues of The Century Illustrated Monthly Magazine, Volume LXXVI, Nos. 1-3, May / June / July, 1908.
