Using a first-person narrative based on her journals, Sakre tells a compelling, vivid and explicit story of the contrasting final stages of two of the most important people in her life: her mother and her father. When her mother was diagnosed with cancer in the 1990s, the doctors took over and led her mother through several treatments that were unnecessary and didn’t give her the cure they promised. She was terminal from the onset, but they wouldn’t admit it. She and her husband blindly followed whatever the doctors prescribed, never questioning them nor did the doctors ever ask them what they wanted. Her mother’s two-year demise was in a morphine haze, totally doctor-directed. Twenty some years later, her dad was given a terminal diagnosis from a collapsed aorta valve. But by then the state of Oregon had the Death with Dignity Act in place, and her dad at age 92 chose to choregraph his own final act by using the law to end his misery. This patient-directed ending was the opposite of what her mother experienced. The two paths are compared side-by-side n the book, so the readers can make up their own minds.
Sakre sets out the stages her parents went through: hearing the diagnosis, finding a “new normal,” living in an interlude, the final endgame and the aftermath of death. The various sections of the book are introduced with quotes from leading authorities in the fields of grief, death and dying. At the end of the book, there is an extensive bibliography for those wanting to delve deeper.
A secondary motivation for Sakre writing this book was the fact that many people told her that they thought it was too difficult to follow the guidelines of the Death with Dignity Act. This ran counter to her experience with her father; they found the steps to be simple and clear. These steps are clearly outlined in the book in everyday terms, so readers can decide for themselves. Finally, Sakre hopes sharing her family’s story with her own parents will be helpful to others walking this path with their own family members or even perhaps for themselves one day.
Being with ones’ parents through their dying process is a gift, and Sakre’s hope is that this book allows the reader not only a glimpse inside her own family dynamics, but perhaps some space to consider their own. She wants people to realize that they have choices; that they should speak up about what they want at the end of their life; and that they don't have to blindly follow what the medical personnel tell them. Above all, if people don’t have a sudden death and receive a six-month terminal diagnosis, they should have the right to decide how their life ends and have supportive people around them to carry out those wishes. To arrive at that point, families need to openly talk about these issues. With that hopeful intention, Sakre trusts this book will help people on their own journeys.
