Critical care transport operates inside the narrowest margin in medicine: the interval between a sending ICU and a receiving one, where a ventilated, vasopressor-dependent, or hemodynamically fragile patient must be sustained with fewer hands, less monitoring redundancy, and an environment — altitude, vibration, acceleration — that acts on an already unstable physiology in ways a bedside training never addresses. This reference connects the gas-law and hemodynamic principles that govern that environment to the organ-system management decisions made during transit, building every chapter on the same foundation: the disease determines the baseline vulnerability, and the transport environment determines whether it is safely navigated or tipped into a preventable deterioration.
What This Book Puts in Your Hands
• Predict altitude-driven device and physiologic behavior — gas-expansion effects on cuffs, pleural air, and VAD flow mapped to specific cabin-altitude ranges and corrective interventions.
• Manage ventilators, vasoactives, and multi-line infusions under transport constraints — mode translation across devices, concentration-mismatch prevention, and battery-reserve planning that eliminates preventable mid-transport failures.
• Recognize the 26 named In-Transit Deterioration Signatures — each pattern names the movement trigger, the earliest detectable tell, the trap logic that leads to misattribution, and the decisive intervention that resolves it.
• Stabilize cardiogenic shock, STEMI, massive PE, and MCS-device patients through altitude and deceleration hemodynamic shifts that bedside protocols do not address.
• Transport the neurologic emergency — ICP waveform interpretation, position-triggered herniation prevention, and stroke-transfer activation sequences that preserve minutes the receiving team depends on.
• Manage neonatal, pediatric, obstetric, burn, and toxicologic emergencies with population-specific dosing, equipment sizing, and transport-adapted clinical decision pathways.
• Apply crew resource management, go/no-go frameworks, and fatigue countermeasures that reduce the human-factors errors transport environments are designed to produce.
Open it before your next mission — the clinical decisions every patient depends on between wheels-up and handoff, in one reference.
