It is safe to say that without Jeffrey Klein, who first described tumes cent local anesthesia, this book would never have been written. This technique of local anesthesia, originally designed for use in outpatient liposuction procedures in the United States of America, was intro duced into clinical practice in Darmstadt in 1990 and is now employed in a growing number of areas of surgical dermatology. Eight years of experience have shown it to be a particularly elegant and safe method of local anesthesia, especially in the treatment of large areas of skin. A large number of scientific studies have improved our under standing of tumescent local anesthesia. The administration of large quantities of a dilute local anesthetic into lipophilic tissue is no longer seen as a risk to the patient, although further discussion at the inter national level will be required over the next few years in order to standardize the tumescent solution. Until then, the widely varying dosages reported around the world should be regarded with caution. The progress made and experience gained in recent years make it likely that tumescent local anesthesia will become firmly established in all branches of surgery. One important factor is the economy of the tumescent technique, the material costs of which are very low. In comparison to general anesthesia, enormous savings can be made in the costs of postopera tive care, owing to reduced staff requirements and fewer complications of wound healing.

Since the introduction of Tumescent Local Anesthesia by Dermatologist and Pharmacologist J.A. Klein, this method of subcutaneous infiltration of large volumes of diluted anesthetic has been more rapidly adopted by doctors all over the world than any other anesthetic method before. Initially invented for cosmetic liposuction, the unsurpassed safety of the procedure has led to many new indications. Today, the advantages are used by dermatologists, general surgeons and plastic surgeons, gynecologists, phlebologists, and ENT specialists. The book contains a wealth of original work and for the first time gives detailed information, clinical examples and tips & tricks on: Medical issues such as liposuction, extirpation of lipomas, wide excision, dermabrasion, flaps and grafts, split skin grafting, hyperhidrosis, acne inversa, breast surgery, sentinel lymph node biopsy. Cosmetic surgery is also covered: abdominoplasty, medial thigh lift, face lift, laser skin resurfacing, hair restoration surgery. Special emphasis was laid on all aspects of phlebologic surgery in tumescent local anesthesia. All contributions were made by internationally renowned experts in their field, such as dermatoloogy, anesthesiology, plastic surgery, general surgery, phlebology, gynecology. The brief but thorough theoretical introduction covers up-to-date background information on pharmacology and technique and makes this book an ideal companion for all courses on the subject.

Issued without a dust jacket. -- A richly illustrated clinical reference that consolidates nearly a decade of German and international expertise in the evolving field of tumescent anesthesia (TLA). Edited by three leading dermatologic surgeons, this volume builds upon the pioneering work of Jeffrey Klein and extends its clinical application beyond liposuction into a wide array of dermatologic, reconstructive, and surgical procedures. It combines detailed pharmacological theory, practical techniques, and multidisciplinary case applications, making it an essential guide for surgeons, dermatologists, anesthesiologists, and medical professionals exploring safe, cost-effective alternatives to general anesthesia. Divided into three parts (Theory, Practical Application, Indications), the book offers an integrated approach to understanding and utilizing TLA. Part A opens with a thorough overview of the technique's origins, its European adaptation, and its evolution from a niche anesthetic method to a broadly applicable surgical tool. A particular strength of this section is its detailed exploration of solution composition. Multiple recipes for tumescent solutions are discussed, with an analysis of the pharmacodynamics of key components such as lidocaine, prilocaine, epinephrine, buffers, and anti-inflammatory agents. Pharmacokinetic considerations - onset, duration, systemic absorption, blood levels, toxicity thresholds - are presented with scientific rigor. The inclusion of clinical toxicology, sedation strategies, infection prevention, and thrombosis prophylaxis contextualizes the method within broader perioperative safety frameworks. Part B delves into the infiltration and infusion techniques with precision. Manual vs mechanical methods are contrasted, and the dynamics of solution distribution are explained in terms of volume, depth, cannula choice, and flow rate. The discussion extends to sedation protocols and patient selection, identifying contraindications and necessary safety preparations. This section supports clinicians with step-by-step technical guidance and offers a scalable framework adaptable to various facility types - from high-volume surgical centers to office-based practices. Part C is devoted to the clinical indications for TLA, which have significantly expanded due to the technique's safety profile, intraoperative comfort, and excellent hemostatic properties. 35 chapters cover procedures ranging from liposuction, abdominoplasty, medial thigh lifts, and facelifts, to more specialized applications like phlebectomy, sentinel node biopsy, arthroscopy, acne inversa, and pediatric dermatologic interventions. Each procedure includes standardized technique descriptions, specific advantages, and limitations, making this section a practical resource for procedural planning and patient counseling. The text argues convincingly for TLA's cost-efficiency compared to general anesthesia, citing lower material costs, reduced recovery times, and decreased incidence of postoperative complications. In the context of modern surgical practice, where minimally invasive procedures and outpatient efficiency are increasingly prioritized, Tumescent Local Anesthesia stands out as a practical, evidence-based, and forward-looking reference. With over 130 figures and clear, methodical presentation, it bridges theoretical pharmacology and hands-on procedural detail, equipping readers to adopt and refine TLA for a wide spectrum of indications. As such, it will remain a valuable tool in advancing safe, patient-centered anesthesia protocols across medical disciplines. Seller Inventory # 011332
