This study explores the entire medical and legal implications of vacuum extraction as a means of accomplishing obstetrical delivery. In the past 20 years, a rethinking of techniques for operative vaginal delivery has occurred and the complex forceps operations of a prior generation are now infrequently performed. Vacuum extraction has become an increasingly popular technique for most instrumental deliveries. The major issues concerning vacuum extraction are the same as those encountered with forceps operations: case selection, evaluation of foeto-pelvic proportions, accurate measurement of position/station, correct instrument placement; and the judicious application of force.
