About the Author:

William A. Nelson, MDiv, PhD, is director of Rural Ethics Initiatives and an associate professor of community and family medicine at Dartmouth Medical School and the Dartmouth Institute for Health Policy and Clinical Practice. He also is an adjunct associate professor of public administration at New York University's Robert Wagner Graduate School of Public Service.
Paul B. Hofmann, DrPH, FACHE, is president of the Hofmann Healthcare Group in Moraga, California. He has served as the executive vice president and chief operating officer of the Alta Bates Corporation, a diversified nonprofit healthcare system in northern California; executive director of Emory University Hospital in Atlanta; and director of Stanford University Hospital and Clinics in Palo Alto.
