Liberation from stigma is the first step in freeing from addiction. Historical and clinical scientific considerations of alcohol, cocaine and stimulants are reviewed. As an example of stigma, E. O'Neill's master-piece playwright "Long Day's Journey Into Night" is given. Primary, secondary and terciary prevention of addiction with evidence based pharmacological treatment of alcohol, cocaine and other stimulants and opiates addiction is discussed. Especial attention is given to simultaneous and combined pharmacotherapy of addiction and co-occurring disorders. The new Portugal law which decriminalizes addiction and captures liberation of stigma is reviewed. Ending with the movie "Flight" which embraces the concept of freedom from addiction and stigma.

Dr. Cesar A. Fabiani was born in Cochabamba, Bolivia. He graduated from the Catholic University, School of Medicine in Cordoba, Argentina at age 23. Dr. Fabiani is licensed to practice medicine in Argentina, Bolivia and in the states of Pennsylvania and New Jersey in the USA. At present he practices out patient psychiatry in Pennsylvania. He is the medical director of APM ( Association of PuertoRicans in March) also part time psychiatrist at Citywide Community Counseling Services in Philadelphia and Internet Behavioral care in Trevose, Bucks County, PA. He is board certified in Psychiatry, Administrative Psychiatry, and Addiction Psychiatry by the American Board of Psychiatry and Neurology. Dr. Fabiani in 2006 was bestowed as Distinguished Life fellow of The American Psychiatric Association. He has written more than 30 articles and 2 books one in Spanish "Familia and Frontiers" and the other " Humanity and Byberry Hospital".
