From the Author

A unique book for case managers in almost any setting.
This book is very different from other case management or nursing texts because it covers various tools for measuring the outcome of health interventions. Outcomes are not defined by the provider, but by the customer. The outcomes can be cost-related, if the customer is the payor, but are most likely the functional health status of the patient that recieves the care. The book covers case management from the outpatient perspective, focusing on early identification of patients that may not do well in the health care system, and how to pro-actively optimize their recovery. The book is heavily documented, with excellent sources for further reading and research, but is a quick and easy read for those that are looking for new approaches to tough case management situations. A comprehensive appendix includes a glossery that covers legal, insurance, quality improvement, utilization review and vocational couseling definitions useful to case managers in almost any setting.
