SIGNED BY HARVEY CUSHING (on the half-title): "Albert S. Crawford/ with warm regards/ H C/ April 8 1939" (see photo). The recipient Alfred S. Crawford wrote "Property of/ Alfred S. Crawford" on the front flyleaf (see photo). This is one of 100 copies on Winterbourne paper, for the members of the Harvey Cushing Society, of which Alfred S. Crawford was a member. The binding--of paper-backed boards, with a printed paper spine label (see photo)--is different from that of the 400 copies for sale, which was bound in a green cloth. In addition the title page imprint of the edition for sale is "Charles C. Thomas", whereas the title page of the edition printed for the members of the Harvey Cushing Society says "Printed for the Society". As for Cushing's inscription, he invariably signed "H. C." for those he knew. He wrote out his name "Harvey Cushing" when inscribing for those he did not know. Seller Inventory # 15868
