Deuxième édition, la plus recherchée, la première à comporter le célèbre récit autobiographique d'Alexina B. (Herculine Barbin), pseudohermaphrodite masculin dont le sexe avait été méconnu à la naissance. Cet ouvrage est ainsi divisé en deux parties. La première partie est consacrée à l'étude médico-légale sur les vices de conformation des organes sexuels de l'identité du sexe comme cause de nullité de mariage ; Des erreurs dans l'inscription du sexe à l?état civil ; Caractères et circonstances qui doivent guider l'intervention médico-légale (examen des organes -pénis, scrotum, urèthre, testicules-, examen de la physionomie et de la constitution physique et morale, circonstances de l'intervention médico-légale) ; Choix d'observations particulières. La seconde partie (pp. 61-174) correspond à l'histoire d'Alexina B., avec la retranscription "presque complète" du manuscrit que rédigea ce jeune homme avant de mettre fin à ses jours. Considérée de sexe féminin à la naissance, Alexina fut déclarée de sexe masculin (devenant Abel Barbin) une vingtaine d'années plus tard. Ce récit autobiographique, intitulé "Mes souvenirs", avait été remis à Tardieu par Régnier qui pratiqua l'autopsie. Il fut réédité par Michel Foucault en 1978. Ambroise Auguste Tardieu (1818-1879) fut professeur de médecine légale à la Faculté de médecine de Paris, membre de l'Académie de médecine, médecin consultant du chef de l'Etat. Il fut un médecin légiste très réputé.
