In recent years the long-standing debate over the nature of mental disorders has intensified. Clinicians insist that mental disorders are diseases and should be treated as such. But a growing number of scholars-from Ruth Benedict and Thomas Szasz to Erving Goffman and R. D. Laing-argue that mental "illnesses" actually are culturally conditioned deviant social roles. In this unique cross-cultural study, John Marshall Townsend evaluates two contemporary social-role theories and attempts to reconcile them with the clinical theories. Townsend explores the two social-role theories through a cross-cultural experiment that compares matched samples of German and American mental patients, mental-hospital staff, and high-schoolstudents. His data support the view that cultural conceptions of mental illness do help to mold deviant behavior, although not in the manner proposed by Thomas Scheff. Townsend's research also suggests a revision of Goffman's classic theory of the relation between the symptoms of mental illness and the requirements of life in mental hospitals. Although his study focuses primarily on the social-role approach to mental disorders, Townsend does not intend it as a repudiation of the clinical viewpoint. Rather, he argues that the impasse between the clinical and social-role positions is the result of a false dichotomy between biology and social learning; this he attempts to reconcile by utilizing an integrated psycho-physiological conception of "social role." In a penetrating conclusion, Townsend maintains that the problems confronting the future of mental-health care will remain insoluble until we recognize the pervasive influence of our basic cultural values and assumptions.
