Oblong, approx. 6 in. x 4 in. Cardboard construction cover with bent nails for closers. Contains an accordion-style fold-out with musical notations. The covers are corrugated cardboard. The front panel has six small "windows," each containing in magic-marker a letter of the word "TWELVE." The inside of the front panel contains a portion of a page which appears to be a listing of titles and authors. Ted Milton (born 1943) is an English poet and musician, best known for leading the Blurt, an experimental jazz-rock group.
