Despite these screening efforts, carcinoma of the cervix remains one of the more common malignancies in women and it is the leading cause of cancer death in many countries in the western hemisphere. It is estimated that in 1986 there will still be 14,000 new cases of invasive cancer, with 6,800 deaths in the United States alone. Unfortunately, many of these patients present with advanced disease, posing difficult management problems for the clinician responsible for their care. The treatment of early stage invasive carcinoma of the cervix (lesions confined to the cervix and vagina) remains either radical surgery, radical radiation therapy or a combination thereof This approach is extraordinarily effective in the vast majority of patients. However, there remains a subset of patients with early stage disease that are at high risk for recurrence. Dr Kjorstad (Chapter 2) has identified adenocarcinomas and adenosqua mous carcinomas as having a particularly poor prognosis. In addition, patients with more than three positive lymph nodes or with involvement of lymph nodes outside of the pelvis have a very poor prognosis. He has iden tified the CEA as a potentially predictive marker for these patients with poor prognosis, especially in patients with adenocarcinomas.

Since that auspici ous event, nine international symposia on the role of taurine in biology have taken place. The locations for these meetings have been Tucson (two times), Rome, Philadelphia, Tokyo, Vancouver, Mexico City, Helsinki, and Florence. In 1977, due to the large number of scientists in Japan who were interested in the role of this unique amino acid in biological systems, we organized the Japanese Research Society on Sulfur Amino Acids with the encouragement and financial assistance of the Taisho Pharmaceutical Co., Ltd (Tokyo). Annual meetings have been held, and the membership has expanded from 78 to 414 in 1987; the number of presentations has increased during this time span from 29 to 74. The symposium in Tokyo in 1982, 'Sulfur Amino Acids, Biochemical and Clinical Aspects' [1], was held to celebrate the 5th Annual Meeting of our Society. I would like to emphasize that in Japan we have an active Research Society especially directed to the study of sulfur amino acids. We have published our own semi-annual journal entitled Sulfur Amino Acids. Our society is an inter disciplinary research society since taurine is a highly diversified compound that interconnects physiology, biochemistry, pharmacology, nutrition, and medicine. One exciting fringe benefit of taurine research and the society has been the fostering of contacts with distinguished scientists from many varied medical fields.

MANAGEMENT OF ARRHYTHMIAS IN THE CORONARY CARE UNIT NORA GOLDSCHLAGER, RODERICK WOODS, AND NEAL BENOWITZ With the advent of coronary care units, mortality from cardiac arrhythmias occurring during acute myocardial infarction has been unquestionably and dramatically reduced. In addition, continuous electrocardiographic monitoring has resulted in the recognition of specific arrhythmias related to acute ischemic heart disease and thus to the development of appropriate strategies for their management. This chapter will review certain specific aspects of those bradyarrhythmias and atrial and ventricular tachyarrhythmias that are seen most commonly in acute myocardial infarction. Pharmacologic therapy, including the use of some of the newer antiarrhythmic agents, will be discussed, as will pacemaker ther apy of both bradyarrhythmias and tachyarrhythmias.

Neuware - For a period of some fifteen years following completion of my internship training in clinical psychology (1950-1951) at the Washington University School of Medicine and my concurrent successful navigation through that school's neuroanatomy course, clinical work in neuropsychology for me and the psychologists of my generation consisted almost exclusively of trying to help our physician colleagues differentiate patients with neurologic from those with psychiatric disorders. It now is gratifying to look back and review the remarkable progress that has occurred in the field of clinical neuropsychology in the four decades since I was a graduate student. In the late 1940s such pioneers as Ward Halstead, Alexander Luria, George Yacorzynski, Hans-Lukas Teuber, and Arthur Benton already were involved in clinical studies that, by the late 1960s, would markedly have improved the quality of clinical practice. However, the only psychological tests that the clinical psychologist of my immediate post-Second World War generation had as aids for the diagnosis of neurologically based conditions involving cognitive deficit were such old standbys as the Wechsler Bellevue, Rorschach, Draw A Person, Bender Gestalt, and Graham Kendall Memory for Designs Test.

Neuware - The last volume dealing with lung cancer in this series in Cancer Treatment and Research was published in 1986 and entitled Lung Cancer: Basic and Clinical Aspects. The present book continues the outline of the previous volume by presenting up-to-date information on lung cancer in critical reviews of new important basic and clinical concepts of lung cancer. The present volume has broadened the scope by also including chapters dealing with issues such as epidemiology, prophylaxis, and histopathology of lung cancer. The content of the book thus reflects the increasing awareness of a global disease that is more and more in focus, not only scientifically but also politically. The latter fact results increasingly in changes in health legisla tion, with prevention measures influencing everyday life. The great interest in the disease is natural, considering that more than one patient dies from lung cancer every minute globally. The first chapter is from the Cancer Unit, WHO, Geneva, and describes in detail the epidemiologic features of lung cancer, which is the second most frequent cancer in the world with 660,500 new cases annually; it will soon surpass stomach cancer as the leader. Thirty-one percent of the cases occur in developing countries, where the increase is especially dramatic.

Even in the 'Scuola Salernitana', the oldest medieval medical school in Western Europe, about 1000 years ago it was taught how to improve urine output. The list of known 'diuretica' included herbs, plants, roots, vegetables, in particular asparagus, fennel and carrot. The first diuretic drugs, however, were mercurial compounds. Thus, calomel, mercurous chloride, was initially used as a diuretic in the sixteenth century by Paracelsus, being one of the ingredients of the so-called 'Guy's Hospital pill'. But calomel had a cathartic effect so that it was replaced by organic mercurial compounds. These diuretics were clearly toxic. After the discovery of the car bonic anhydrase, in the early 1930s, and the introduction of sulfanilamide as a chemotherapeutic agent, it was observed that this drug was inhibiting carbonic anhydrase in vitro and urinary acidification in vivo thereby causing metabolic acidosis; urine output, however, appeared to increase. Subsequent studies led to the synthesis of more potent analogs, in particular acetazolamide. Studies on car bonic anhydrase inhibitors led to the synthesis of benzothiadiazides which disclosed much less inactivating action on carbonic anhydrase and much more diuretic effect through an inhibition of tubular transport of sodium and chloride. Chlorothiazide was the first member of this class of diuretics. Thiazides are still used in clinical practice.

Neuware - The theme of this 14th International Symposium on Blood Transfusion is closely related to the work and scientific contributions of the Dutch cryobiology pioneer Dr. Herman W. Krijnen of the Dutch Red Cross Central Laboratory. Dr. Krijnen was known and respected in the national and interna tional blood transfusion community as an extremely competent scientist and a beloved and admired colleague. Dr. Krijnen was intentionally honoured with the invitation to open this symposium on cryopreservation and low temperature biology in blood transfusion and be the guest of honour at this event. Unfortunately, Dr. Krijnen suddenly died on the first of June 1989. In honour and mem ory of Dr. Krijnen this symposium will therefore be dedicated to him. Since the lOth International Symposium on Blood Transfusion in 1985 highlighted the theme of 'Future developments in blood banking', major changes have occurred in the blood banking world. Most of these changes were forced upon the Blood Banks by the fear of spreading AIDS through contaminated donations. This not only led to the wide spread testing of blood, but also to a more appropriate counselling of the community and the blood donors in specific. Additionally, virus inacti vation techniques were introduced for those components derived from multiple donations and intended for a regular transfusion in haemophi lia patients and others.

A central purpose of this study is to gain sufficient scientific information and insight to enable the development of both representative and useful models of the superabundance of physical processes that surround us. The engineers need these models and the associated insight in order to build the information processing systems and control systems that comprise these new and emerging technologies. Much of the early modeling work that has been done on these systems has been based on the linear time-invariant system theory and its extensive use of Fourier transform theory for both continuous and discrete systems and signals. However many of the signals arising in nature and real systems are neither stationary nor linear but tend to be concentrated in both time and frequency. Hence a new methodology is needed to take these factors properly into account.

Congestive heart failure also is one of the commonest causes for hospital admissions accounting for over 5 million hospital days per year. Despite the early recognition of this condition and active medical research into both mechanisms and therapy, prognosis continues to remain dismal wi th less than a 50% expected five year survival. In the last decade we have seen many new medical and therapeutic options for patients with congestive heart failure which extend beyond the use of bed rest, sodium restriction, digitalis and diuretics. These include vasodilators of a variety of types including the angiotensin conventional enzyme (ACE) inhibitors. Also, many new inotropes are under active investigation both in oral and intravenous forms. In March of 1984 a survey of over 5000 physicians was performed under the auspices of the American Heart Association (reported in: JAOC 8:966, 1986). That survey showed that there was no universally accepted defini tion for congestive heart fai lure and that a wide spectrum of diagnostic cri teria for this common condi tion existed even among academic cardiologists. There was no clear standard as to even the mos t bas ic treatment of conges t i ve heart fai lure. For example, exercise restriction was recommended by 19% of physicians, 31% recommended no change in activity, and 50% either light exercise or an exercise conditioning program.

First, by dividing the oncology literature into specific subdivisions such as lung cancer, genitourinary cancer, pediatric oncology, etc. Second, by asking eminent authorities in each of these areas to edit a volume on the specific topic on an annual or biannual basis. Each topic and tumor type is covered in a volume appearing frequently and predictably, discussing current diagnosis, staging, markers, all forms of treatment modalities, basic biology, and more.

ICOP meetings have been held biennially in the USA, Europe, and Japan in turn. The previous three ICOP meetings (7th to 9th) were held in Shimoda, Japan, in 1988. in WildbadKreuth, Germany, in 1990, and in Williamsburg. Virginia, in 1992. Previous meetings were held in resort areas, this was the first meeting held in a large city. Attendance has grown every year so that nearly 400 participants from 19 different countries attended the Tokyo meeting. At the meeting, novel and updated results on the structure-function. physiology, biology, and pathology of proteases and inhibitors were discussed, together with cellular aspects of proteolysis and protein turnover. Thirty-nine invited papers and eight selected posters were presented orally and 171 poster presentations were discussed. This book documents almost all of the lectures and some selected posters. Since the world of proteolysis and protein turnover is expanding very rapidly. far beyond our expectations, it is impossible to cover all the new aspects of this field. However, this book will give an idea of the current status, trends. and directions of the field, and information necessary to understand what is and will be important in this field. Further. the editors hope that the novel ideas, approaches. methodologies, and important findings described in this book will stimulate further study on proteolysis and protein turnover. 324 pp. Englisch.
