Cette synthèse définitive de son travail sur l'anatomie, la physiologie, les lésions et les altérations de l'encéphale offre "un tableau complet de la structure, des fonctions et des maladies du cerveau", elle est destinée "aux étudiants et aux médecins". Scipion Pinel y ajoute des recherches nouvelles sur les "lésions de la motilité", c'est à dire la paralysie générale, "sujet le plus difficile et le plus important des points nouveaux". La « préhistoire » de la psychiatrie brésilienne commence avec l'arrivée de la famille royale en 1808. Fuyant l'expansion napoléonienne, la cour portugaise décida à ce moment-là de s'installer au Brésil, qui était alors la plus grande et la plus riche des colonies portugaises. En tant que colonie dépourvue de tout projet de développement, la société brésilienne était essentiellement de nature agricole, fondée sur des grandes propriétés rurales (latifúndios) dont le fonctionnement était régi par un ordre économique reposant sur la spoliation et l'exploitation du travail des esclaves. Avec l'arrivée de la famille royale et son installation à Rio de Janeiro, devenue du même coup capitale du royaume portugais, un processus d'urbanisation accélérée s'amorce, marqué notamment par la création de diverses institutions telles que la banque du Brésil et les premières écoles de médecine et de chirurgie à Rio de Janeiro et à Salvador. Quelque vingt années plus tard, les premières sociétés de médecine se constituent dans un pays qui a déjà acquis son indépendance : ainsi de l'Académie impériale de médecine et de la Société de médecine et de chirurgie de Rio de Janeiro. À peu près à la même époque, apparaissent les premières revues spécialisées telles que Les annales de médecine, de chirurgie et de pharmacie (revue fondée par Xavier Sigaud, médecin français installé au Brésil) ou encore le Semanário de saúde pública.
