Tipped in is scrip page of "F Ellsworth Bratt, M.D. 1219 E Third St. Telephone 474". As reported in "The Lancet 1911 - Dr Frank Ellsworth Bratt, aged forty-two, of 1219 East Third Street (Chicago) died August 20 of myocarditis. He had been practicing in this city since 1895.
