Items related to Community Care: Managing the Cascade of Change
For many years there has been a gradual shift in the balance of care away from hospitals to care in the community. The Government's White Paper, "Caring For People", addressed the implication for this development, creating a planning framework for the management of community care services. In local authorities the concept of care management is being promoted as the cornerstone of high quality care. The aim is to link users' and carers' needs more directly with the resources available, with providers adjusting their services to meet these needs. However, while this rebalancing appears straightforward, in practice it requires adjustments at every level which in turn trigger further complications in an ever increasing cascade of change. This report examines the problems and identifies the direction in which solutions must be sought with regard to changes in community care.
